For Registration You Must Bring The Following Doccuments:
Proof of Age - Official birth certificate
Proof of Income- To verify your child's eligibility we will need to see the following:
Recent pay stubs and tax returns
Recent notice of Action (within 3 months)
SSI (for parent or child)
Proof of address- A utility bill in your name and address (light, water, gas, rental agreement)
If you reside in someone else's home, you must provide their utility bill and a completed address affidavit verifying your address. (pick up in office).
Immunization record- Child's original yellow card must be presented and stamped by the doctor or clinic.
